Present Dental coverage  Level One Basic - 100% co insurance  Level Two Major and Restorative – 50% co insurance  Both levels have a combined annual maximum of $1250 per person per calendar year  that is to say coverage terminates when the combined liability for level one and two reaches $1250

Proposed change  Add level three-  Orthodontic coverage for dependents up to age 19  Advantage; allows members who are families or couples with dependent children to have Orthodontic coverage. Note; no effect to single experience or premium as are ineligible (if maximum remains the same)

Premium effect  Add level 3 at a combined annual maximum of $ per person per year  Cost – Single$38.75 monthly (existing $38.75) Couple$77.75 monthly (existing $77.50) Family$ monthly (existing $96.75)

Examples (Note proposed plan dependent on arrangement with Orthodontist/Dentist and Patient) Present plan (level three)  Cost of Orthodontics $6,000  No Coverage Proposed Plan (level three- option 1)  Cost of Orthodontics $6,000  Coverage 50% combined annual  year one cost $2200- coverage $1100  year two cost $2200- coverage $1100  year three cost $800- coverage $400  year four cost $800- coverage $400 Note : Each year can be billed, and likely will, costs of other levels

Information: 2012 Dental charges  Exams, cleanings, scaling and fluoride (level one)  $ to $  Root canals (level two)  $ to $  X-rays (level one)  $20.00 to $25.00 each

Status Changes  Please note;  Regardless of the level of coverage under the Dental plan, one can and should keep status of dependents up to date.  Example; upon the loss or acquisition of a dependent; (spouse/child) make the change through an enrollment form, available from the board office, to amend your status. Please do this within 90 days prior to, or after, the change otherwise coverage is limited to $250.00, for that new dependent, for one year
