Note: Refer to your collective agreement or contact MyHR to confirm whether wait periods or eligibility requirements apply. There is a six-month wait period for extended health and dental Who can be enrolled under my plan? Legal & Common-Law Spouse Children under 19 or under 25 if they are enrolled in full-time attendance at school Mentally & physically disabled dependants past the maximum age and became disabled before maximum age and the disability is continuous Dependants residing with former spouse who is not eligible for health and dental coverage

Basic dentistry comprises services that are necessary to restore teeth to natural or normal functioning and include services like exams, cleanings and fillings. Basic services are reimbursed at 100% to plan limits. Major services comprise services required for reconstruction of teeth and for replacement of missing teeth where basic restorative methods cannot be used satisfactorily. Orthodontic services are those services provided to maintain, restore or establish a functional alignment of the upper and lower teeth. Reimbursement is subject to the Dental Fee Schedule and plan maximums. Non-taxable benefit You must enrol to be covered Topics Previous Next

10 Employee Basic Life Insurance Plan
PREMIUM COVERAGE Premium for the first $100,000 of insurance coverage is employer-paid. Employee-paid monthly premium for coverage above $100,000 is 18 cents per thousand dollars. Coverage is equal to three times annual salary or employer-paid minimum coverage ($100,000), whichever is greater. Includes accidental dismemberment insurance, loss of sight insurance, and a terminally ill advance payment. Taxable benefit Automatic enrolment upon meeting eligibility requirements Basic Life Insurance: Mandatory enrollment; automatically covered when employee is eligible Benefit amount is 3 times annual salary Employer paid premiums are a taxable benefit to the employee An advance payment of up to $50,000 or 50 per cent of the employee life insurance, whichever less. Important – designate a beneficiary (benefit payment to the beneficiary is non-taxable) or it defaults to your estate Employer covers premiums for up to the first $80,000 of coverage – employee pays the remaining premium You can also choose to enrol your spouse or dependent children in Optional Spouse & Dependent life insurance, which pays a benefit of $10,000 in the event of the Death your spouse and $5,000 in the event of the death of a dependant Child. Enrol online using Employee Self Service (ESS). You can fill out a Beneficiary Designation form on ESS and then print, sign and mail in. 15 Enrolment Deadline MSP & Extended Health/Dental
You can enrol at any time. You are responsible for ensuring you are enrolled for any of the benefits plans you wish to participate in. Coverage is effective the latter of: date you became eligible; or 1st of month following the date your enrolment forms are received. 18 How to make a claim Extended Health/Dental
Online -> GroupNet for Plan Members. Pay Direct -> pharmacy, dentist etc. Manual claim -> Claim forms on GroupNet or MyHR. Complete form, attach receipts and mail to GWL. Don’t miss out! Claim as soon as possible after purchase. Life insurance -> Contact MyHR. They will initiate the claim. Topics Previous Next
