A Community Information Management System by Pangea Foundation

“Ensuring that youth who age out of care have the means and motivation to become successful adults requires innovative approaches to engage and prepare these youth for life on their own. New partnerships among the state and local systems of public education, workforce development and social service that can leverage services and mobilize resources across communities must be formed.” Youth Transition Action Teams Leveraging Community Resources to Ensure Successful Transitions for Foster Youth (Foster Youth Transition Network), 2005 Page 2

The Chafee Act includes mechanisms for system collaboration designed to improve independent living services, but it is up to individual communities to determine how to collaborate effectively at the local level. Youth involvement in planning processes is also mandated by law: “The certifications section of the Foster Care Independence Act of 1999 directs states to include a broad range of stakeholders in the planning, coordination and delivery of independent living services...”

it’s my life a framework for youth transitioning from foster care to successful adulthood “Developing collaborative processes across organizations is challenging, time consuming, and stressful. Foster youth receive better quality services when providers are working in a collaborative manner.”
