 physical examination  medical history  doctor’s office  immunization  health screening  medicine  over-the-counter medicine  prescription

 Immunizations are required to attend school in the United States.  The School Nurse, Social Worker, Psychologist, or Counselor can give you a list of doctors and dentists in your area as well as inexpensive insurance for children.  A physical examination is required before participation in any school sports activity (before entering 6 th Grade).

 Students may take medicine at school.  Parents MUST take the medicine and note (or doctor’s note) to the office.  Students may NOT bring medicine to school themselves.  Only the School Nurse can give the medicine to the student (including Aspirin)  Students must have a pass to see the Nurse.

 Every kindergartner or new student to Prince William County Schools is screened for hearing and vision.  Screenings are given in grades: K, 3, 7, 10  Parents or teachers may request hearing or vision screenings at any time.  An eye exam at school is not the same as a prescription from a doctor.

Read and discuss:  Book Level “Literacy” (Green) [“My Head Itches”] p. 8-9; complete p. 10 and the bottom of p. 11  Book Level 1 (Blue) [“An Eye Exam”] p ; complete p  Book Level 2 (Pink) p ; complete p. 12, 14

 Write a note to bring to school so that your child may take medicine for a bad cough.  “Practice” calling a doctor or dentist to make an appointment.  Explain the difference between medical forms.

 Use the Internet to find local doctor and dental offices.  Type a letter to bring to school with medicine for their child to take at school.
