Work Term Documents The Work Term and Audit Packages are found in PlacePro under “Online Forms & Information” Successful completion of Work Terms/packages are required to graduate with Co-op designation Audit Package(s) may be submitted only when required work terms are completed Returned to you for your records

Activity and Reflection Logs Completed weekly Complete both sections Use “ Skills and Qualities” list to help you identify learning outcomes

Interim and Final Performance Appraisal Signed/dated by you and your supervisor All sections completed Keep copies of these Appraisals for your portfolio
