Supplemental benefits provide… Benefits paid directly to you – not your hospital; not your doctor. Benefits are paid regardless of any other benefits you may have with other insurance companies. Benefits that do not coordinate, integrate or offset with other sources of income or other coverages. (What you see is what you get) Benefits paid to help offset out-of-pocket expenses and help pay for everyday living expenses. Benefits paid for cancer screening and wellness checks for early detection. (Colonial pays on multiple policies)

Key features of Disability This plan replaces a portion of your income if you become totally disabled due to a covered off job accident or sickness.. Plan design… Up to $6,500 per month / up to 60% of income Select from 3 or 6 month benefit period (provides financial protection before your LTD starts paying) Waiting period starting at 0 days for Accident and 7 days for sickness Partial/residual disability benefit

Key features of Accident This plan provides lump sum benefits to help offset unexpected medical expenses due to an on or off job accident. Optional spouse disability rider available up to $1,500 per month. Dependent Care Flexible Spending Account Elect a specified amount up to $5,000 per plan year to be subtracted from your gross earnings before payroll taxes are calculated. Use the money to help pay for qualified dependent care expenses. “Use it or loose it” rule: IRS rule requires you to forfeit any unused funds remaining in your FSA account at end of eligible claim year.
