3 Medical Plan Changes Kaiser $10 co-pay plan is ending on December 31, 2017 195 of us are on this plan 195 MUST change plans Still have Kaiser PPO plan and Regence 250 plan Adding new 3rd plan option for January 1, 2018: Kaiser 200

4 Definitions Network: The facilities, providers and suppliers your health insurer has contracted with to provide services. Co-insurance: Your share of the cost of a covered service, calculated as a percent (for example, 10%) of the allowed amount for this service Copayments: A dollar amount (for example, $20) you pay for a covered health care service, usually when you receive the service. Deductible: The amount you owe (for example, $250) for covered health care services before your health insurance begins to pay. The deductible may not apply to all services. 4th Quarter Carryover: Any portion of your annual deductible that is met during the last three months of the calendar year also applies to the deductible for the following year.

8 Pick your medical plan for 2018
If you are currently enrolled in the Kaiser $10 copay plan AND You want to be on the Kaiser 200 Plan Print and sign your name on the Kaiser 200 list in HR or at the optional evening meetings If you do not make a selection by November 17th you will be defaulted to the Kaiser 200 plan You want to be on the Kaiser PPO Plan or the Regence Plan Fill out a full enrollment form to make the change If you are currently enrolled in the Kaiser PPO plan or Regence plan You want to switch to a different plan You want to stay on that plan No need to fill out an enrollment form

9 Do you waive medical coverage for your spouse or child(ren)?
Bring in a copy of their insurance card to HR to verify other coverage Are you adding or dropping your spouse or child(ren) to/from your health benefits? Fill out a new enrollment form for 2018

10 Other Plan Updates LTD(non-CDL positions): Dental remains unchanged
VSP Vision: Increase in annual frame allowance from $120 to $150 on first and second pair LTD(non-CDL positions): Increase benefit maximum from “to age 65” to “Social Security Normal Retirement Age (SSNRA)”

Tax deferred Total annual election amount deducted in equal increments from each paycheck throughout the year Health Care FSA Maximum Amount: $2500 Common Eligible Expenses: Prescription drugs, copays and coinsurance, deductibles, office visits, dental work, orthodontia, chiropractic, massage etc… Day Care FSA Maximum Amount: $5000 Common Eligible Expenses: Child care, preschool, before and after school care, day camps etc… Does not automatically re-enroll. You MUST re-enroll each year to keep this benefit

Colonial Supplemental Benefits -Brian Burkhart, Benefits Counselor Contact phone: Colonial Supplemental Benefits includes a variety of benefit choices to suit your needs. Disability Insurance - Replaces a portion of your income if you miss work as a result of a covered accident or sickness Accident Insurance - Helps offset unexpected medical expenses that can result from a covered accident. Cancer Insurance - Helps offset the out-of-pocket medical and indirect non-medical expenses related to cancer diagnosis and treatment, including benefits for specified cancer screening tests. Wellness Program Participation saves you $35 per month on your medical premium contributions. Required: Register for Jiff Complete Health Assessment Print or completion certificate Covered spouses must also complete required steps Employees invite spouses by entering while signed in to Jiff Employees and spouses get a $35 reward for completing required steps Deadline: November 17th
