12/9/11 ISP Forum Questions and Answers about the ISP Process Revisions 1

Questions & Answers Is there still a rule about preparing for the ISP within 45 days prior to the meeting? –No. There is no longer a 45-day requirement for starting preparation work. Focus on partnering with other team members, completing the required Pre-Meeting tasks and sharing them with the team no less than one week prior to the ISP meeting. 2

Questions & Answers Is a new RTR cover sheet needed every year? –Yes. A new RTR cover sheet is completed as part of the Pre-Meeting tasks every year. 3

Questions & Answers Is the new Action Plan for People Receiving Employment/ATE Services the only action plan that employment/ATE providers should use? –No. The new Action Plan is specifically to address the person’s Path to Employment and steps that are being taken to move the person toward employment. 4

Questions & Answers Can the exception box be used to not receive "approval" instead inform the other members of the changes? –Yes. That is the way the exceptions box was designed. However, all exceptions must be decided by the entire ISP team and agreed upon by the entire ISP team. One provider cannot impose only their own expectations for how approval will be obtained. Talk about this issue within your ISP teams and make agreements that work for everyone. 6

Why Employment for Individuals? Why Employment for Individuals? 13 Continued from previous page We understand that the current version of the Employment/ATE Action Plan does not allow you to add additional lines of measurable steps. In the short term, please add additional pages but feel free to note “continued from previous page” (or something similar.) OTAC will work on a longer term solution to this issue.

Specific changes related to Employment/ATE Services There is a new required Action Plan for People Receiving Employment/ATE Services which must be included in the ISP for every person in DD 54 service element. The ISP Service Supports page has an updated list of types of Employment/ATE services. The ISP Meeting Agenda now prompts the ISP team to consider the person’s Path to Employment. The ISP Meeting Agenda prompts the ISP team to reflect on at least one specific accomplishment within the last year at Employment/ATE (as well as Residential services.) Weekly schedule written on the ISP should be the “optimal” weekly schedule as defined in the manual. 17

Partnering with Therap OTAC is pleased to partner with Therap Services in 2012 as they develop a first draft of Oregon’s ISP forms in their system. – 18

Timelines For ISP Meetings held on or after 1/1/2012 Must use the new ISP meeting agenda, ISP Service Supports page, and ISP signature page. All individuals receiving Employment/ATE services must have a new Action Plan for People Receiving Employment/ATE Services included within their ISP at their 2012 ISP Meeting. Any change that occurs after this date must use the new change form. All other ISP forms and materials (dated ) may continue to be used. 19
