The project addresses three challenges unwillingness to retire or live in the old age absence of connection among sectors responsible for elderly and among the elderly themselves and lack of responsiveness or inclusion of elderly in the programs of active aging too big of a burden and not enough qualification of informal caregivers of elderly at their homes All three challenges cause health inequalities among the elderly and among the elderly and the rest of the population in the long run.

The project goal is to reduce the differences among the users, and specifically develop programs for the preparation for old age, establish intersectoral support network for elderly care and their inclusion and reduce the differences in the quality of home care in comparison to institutionalize care and with that among the users. The preparation for an active and healthy life in the old age We are developing a training model for multipliers to promote the principles of active and healthy life in the old age. Target group; People in the period before the retirement between the ages of 50 and 65 years.

Healthy, active and included elderly in the rural environment We will develop programs and services, which will be implemented within then network based on the resources of already existing structures of local community. We want to increase inclusion of elderly in the programs for active ageing, reduce isolation in within ensure a longer and quality based living at home. On the level of local community pilot Clubs 65+ will be established, which will activate and integrate local resources for implementation of health, social and other programs and services for target groups in the local environment.

Organizing comprehensive help for elderly (healthy, chronically ill elderly, ill, disabled and their families) at home We will develop and test a pilot model of health and welfare which will enable longer residence in home environment and improve the quality and offer of the care, health care and social work service in the field of care for the elderly at their home. We will develop and test the skill set and its dissemination among expert workers - multipliers, who will disseminate those skills among formal and informal caregivers

Organizing comprehensive help for elderly (healthy, chronically ill elderly, ill, disabled and their families) at home To improve the possibility of home rehabilitation we will establish a pilot mobile multidisciplinary rehabilitation team and test the mobile rehabilitation service at home.. (tele- rehabilitation?!) Target Group; informal caregivers (relatives and other lay providers), who care for elderly at home Expert workers in contact with the elderly and work in the field of public health, basic health and social care Ill elderly and elderly with special needs with no institutional care
