JSIA Mustang Pride Before and After School Program 1333 Marsh Avenue Fort Myers, FL Telephone Fax All counselors are employees of the Lee County School Board. Jackie Smith Director

Program Description JSIA Mustang Pride is a service provided by the school for students in Kindergarten-5 th grade. We provide a safe and caring environment for your child to interact and participate in enrichment activities. JSIA Mustang Pride will not be in operation on days the students are not in school, such as holidays or teacher in service days. We will not offer afternoon childcare on early dismissal days. Discipline Policy If any child receives a discipline report from JSIA Mustang Pride, he/she could be subject to suspension. You may be asked to attend a meeting concerning your child’s behavior. In certain situations, your child can be suspended immediately or could be dismissed from JSIA Mustang Pride. If there is an issue with discipline, a report will be completed and must be signed by the parent/guardian. Accidents In the event of an accident resulting in an injury, JSIA Mustang Pride is not responsible for any medical expenses incurred from injury. All accidents are documented and must be signed by a parent/guardian. The supplemental insurance, which is offered to each child through the Lee County School System, will cover your child during these hours. Medication If your child needs to take medication while attending JSIA Mustang Pride, the proper forms will need to be completed by the parent AND on file with the school clinic. Medication must be in a labeled container with the correct name and dosage. Please see the director or assistant director if your child requires medication while attending JSIA Mustang Pride.

Snack Time A healthy snack will be provided to each student after school (no snacks may be brought from home). If your child attends our before school care program he/she can eat breakfast in the café prior to going to class. All allergy information regarding a student must be provided by the parent and on file with the school clinic. REGISTRATION There is a one-time Registration Fee of $10.00 per child. FEES Before School Hours 7:30 am – 9:10 am Fee per morning $5.00 per child No child is to be on campus prior to 7:30 am. Students may not bring food to eat at school. This includes breakfast and/or snack. After school Hours 3:50 pm – 5:30 pm Fee per afternoon $8.00 per child

Payments Payments are due before services are rendered. Payment for program services is due on the Monday for that week. If paying by the day, the money is due the morning of, prior to the child staying. If payment is NOT received by MONDAY for the week, the child will be unable to attend. Parent/Guardian will be contacted to pick child up. All payments are to be made promptly. The director and/or principal has the authority to revoke the enrollment of a student in the program, if any of the following situations occur: 1) Non payment of regular weekly fees 2) The student is picked up after 5:30 pm on a regular basis OR 3) The student receives more than three discipline referrals. ***Students will not be permitted to attend JSIA Mustang Pride unless their account is kept current (including payment of any late fees, etc). Drop-Off Policy Students are to be brought into the school, via the side gate, and checked in by a JSIA Mustang Pride staff member in the media center. Students may not arrive earlier than 7:30 am. Gate/Door will not be opened until 7:30 am. NO STUDENT CAN BE DROPPED OFF WITHOUT A PARENT/GUARDIAN CHECKING THEM IN. Pick-Up Policy Anyone picking up a child MUST be 18 years old, present photo identification and be on the child’s registration form. Please notify us of any changes in writing. There is a late fee of $15.oo plus $1.00 per minute per child for children picked up after 5:30 pm. If a student is not picked up by 5:30 pm, we will make every attempt to contact someone on the registration form, the emergency card or school database. If we cannot contact anyone following a reasonable period of time, a school administrator will be contacted. We will then call the Lee County Sheriff’s Department.
