Comprehensive Program Review March 16, 2012

 Successfully opened two new programs, both doing very well.  Danielle’s program is now running very smoothly, mom is extremely happy.  Dina went to Florida for the first time in her life, stayed 3 months and had a fantastic time!  Ian is now expecting more support hours, he is feeling comfortable with staff and is enjoying life more now.  Josh was finally able to find employment with staff’s assistance, after many months of searching, he is employed at Price Chopper and loving it.  Three of our autistic individuals (Ericka, Jenny and Danielle) are attending a gymnastics group for adults with autism and they are doing very well.  New program in New Market and filled apartment in New Market site.  Increased referral and work with Autism.  Successfully worked with Provider, Family and Individual through Cancer diagnosis and treatment.  Need to hire a Program Coordinator for Keene.  Continue to stabilize the Andover St. Program.  Continue to work on a more seamless process for RFP’s and intake.  Resolve the issue of Day Program space at 1480 Elm St., Manchester.  More timely evaluations. Data obtained from CSNH

 There has been a 32.5% increase in clients experiencing new day activities since last CPR (40  53).  Only 1 restraint occurred August-January.  There were 0 elopements August-January.  There were 0 incidents involving contraband August-January.  An average of 28% of clients were competitively employed (August-January).  CSNH received 3 written commendations from site visits by outside regulators/funders and 27 deficiencies (over 6 months). However, at the time of the last CPR there were 38 deficiencies so this is an improvement.  The number of medication incidents and occurrences rose from 1 (at the time of the last CPR) to 7.  There is a steady decline in the percentage of department success with individuals community involvement.
