UNOS Region 5 Spring Meeting March th 2015

3 Process Flow was Born Several years ago we were in the process of establishing a Process Improvement Opportunity, the teams started brain storming on a weekly basis, flow charting ideas, tossing them around to see what would stick. We officially called this our “Process Flow Meeting”. In the beginning we met weekly and then it went to monthly. The participants are: Medical Director Transplant Nurse Coordinators Social Workers Financial Coordinators and Management Assistants Etc.

4 How to speed up the process of getting a patient through evaluation and listed timely? The one question…….

Second Question? Could we evaluate a straight forward patient in One Day and then present in Selection Committee the following week???? 5

Answer………….. 6 Yes, we thought we could but…. What would be the hurdles? What are the Pros and Cons?

7 Pros and Cons Cons  Long Day for the patient  Multiple appointments through out the medical center  Keeping patients on track from appointment to appointment during the day.  Payer contracting for Evaluation Services. Pros Patient Satisfaction Increase waitlist Marketing point for payers and referring providers Evaluation services easily tracked for inclusion on the Cost Report Organized process of getting the patient from referral to listed Group Patient Teaching

How to deal with the CONS 8 Long day: 1. Ancillary and Diagnostic Testing in the AM 2.Group Patient Teaching w/Lunch (Separate one for Spanish Speaking) 3. Consults in the afternoon Multiple Appointments and Keeping Everyone on track: 1.Worked with ancillary departments to establish block times on a specific day 2.Created a transplant ambassador program- Post patients that escort the patients to their appointments. Keeping everyone on time and abreast of any delays. Contracting Issues: 1.We worked with the Payers and Medical Groups.

The Pros……. 9 The Pros out weighed the cons….

A Snap Shot of our “ One Day Evaluation” 10 AM: Labs Imaging Cardiac Testing (EKG, Stress Test, Etc.) Imaging (Chest X-Ray, CT or MRI Pulmonary Function (if Needed) Noon: Lunch for Patient and Care Giver Patient Teaching PM: Dental Nutrition Social Worker Financial Coordinator Surgeon Specialty Consults as needed (Cardiology, Infectious Disease, Anesthesia, etc.)

In the end….. 11 Our patient satisfaction increased, the staff and physicians were happy. And Today it is all very routine.
