CQI PROJECT 2013 CERVICAL CANCER SCREENINGRISK REDUCTION COUNSELING

Cervical Cancer Screening  Plan: Train newly hired nurse clinician to perform screening  MCM staff to perform chart reviews prior to client visit  Do: Gyn nurse to offer screenings on a daily basis as well as scheduled nurse Gyn visits, in addition to Gyn services provided by Ob Gyn M.D. every other Tuesday and every Thursday.

Cervical Cancer Screening  Study: After three months look at the % of women who attended clinic that agreed to have cervical cancer screening, and the % of women scheduled who attended cervical cancer screening.  Act: Continue this process ( goal is for 80% of women to receive cervical cancer screening)

Risk Reduction Counseling  Plan: Meet with clinic staff to review performance. QM education for staff.  Do: MCM staff and Gyn nurse to enter RR counseling, data clerk to enter RR counseling in CHAMP for gynecologist and ID physicians  Study: After three months look at data in CHAMP for % of patients who receive RR counseling.  Act: If successful continue this process if not, reevaluate and implement a new plan
