Reporting/Scheduling  New Employees  Department determines if employee needs to process through OHS via Assessment Tool on OHS website  Department completes New Employee Routing Form on OHS website for employees who need assessment at OHS and submits online  New employee calls to schedule new employee health screening.  Work Injuries  Employee or Supervisor: Complete online Work Connections Illness/Injury Report Form, print prior to submitting and have employee bring copy to OHS clinic
