2 Unitholder/Credit cardholders complete Application Form and forward to MAAKL Mutual’s Offices or Service Centres Operations Dept. will verify and process Application Form Enrolment and Billing file CIMB Bank will verify cardholders’ unique information with Visa or Mastercard. Merchant Bank Visa or Mastercard will debit the payment from the cardholder’s merchant banks 54 Payment received by MAAKL and credited into Unitholder’s A/C 3

4 Guide for Filling up 1.For Non-existing account holder’s with MAAKL, you are to attach the Fund Application Form together with the application form, photocopy NRIC and photocopy credit card. 2.For existing account holder’s with MAAKL, you are required to submit the Application form duly completed together with photocopy NRIC and photocopy credit card. 3. Application Form must be witnessed by UT Adviser or MAAKL Mutual staff. You are advised to read the terms and conditions before filling up this form.

5 MAAKL’s Terms and Conditions 1.Photocopy NRIC and credit card must be attached together with the Application Form. 2.Minimum amount for is RM per transaction. 3.Maximum amount for is RM5, per card and transaction. 4. will commence 21 days from the date of submission of forms to Operations and Administration Department, MAAKL Mutual. 5.The investment date for will be either on 10th or 28th of each month. 6.If the investment date happens to be a non business day, the next business day shall be used to credit such payment into the unitholder’s account and the price will be based on the valuation of the next Bursa Malaysia business day. 7.If the debiting is not successful due to the credit card’s record are not in order / active eg. Card expired, MAAKL will not take any responsibility for such case unless the bank is able to deduct the payment successfully on the investment date.

6 MAAKL’s Terms and Conditions 8. In the event that a deduction is unsuccessful, MAAKL Mutual will write to inform the unitholder. 9. Any successful payment shall not be refunded for whatever reason. 10. The manager reserves the rights to terminate the for payments that are unsuccessful. 11. Any cancellation or change of, the unit holder will have to advise MAAKL MUTUAL in writing. 12. Not applicable for investment into Bond and Money Market asset class, MAAKL Asia-Pacific REIT, EPF Members investment Scheme Account and investment purchased at NAV without sales charge. 13. MAAKL Mutual Bhd reserves the right to change the above terms and conditions at any time without prior notice at its sole discretion.

However, the Fund Managers reserve the right to terminate your request for payments that are unsuccessful.

No, the Unitholder or the Credit Card Holder can only cancel the via MAAKL Mutual in writing at least 1 month before the next investment due date. 6.Do I need to submit multiple application forms to debit from a single credit card account number for different investment accounts at MAAKL MUTUAL? Yes, you are required to submit more than one application form if you wish to debit from a single credit card account number for different investment accounts. However, you must clearly state MAAKL’s account number that you wish to credit to, on the form. 7.Whose account can I invest for? This is applicable for the account of the cardholder’s spouse, children and the cardholder’s own account(s) only.
