Required documents to be submitted w/ the Level II application  Credential request form  Receipt of credential fee payment  Copy of your web-portal transcript  Copy of a valid CPR Certificate for Adults/Child/Infant  CL41-Exp form –verification of 2 years full time education specialist experience Must turn in original signature & form must be verified by a credential technician or Certificated HR personnel in your school district.
