 If it is not recorded, it didn’t happen  Record everything  Send in evaluations EVERY WEEK  Grant money is tied to evaluations  We provide 30 copies of all forms per class  First set is in your box  Extras are on-line  Notify your project manager well in advance of your 2 nd, 3 rd, 4 th classes so we can send your next set of evaluation forms

 Throw away all old forms!!!  Do not use evaluation forms on the Smart Steps CD.  Each class has a unique identifier (number)  Discard anything that has an old class number  Only use the new envelopes to mail the evaluations  Use the new attendance forms

 Pink paper in tri-fold  Taped onto individual folders in your boxes  On-line  Pre-program paperwork (20-30 minutes)  Consent letters (adults & children) – √ signatures  Confirmation of Head Start Eligibility (OHS only)  Pre-questionnaire  Domestic Violence fact sheet  Attendance

 Send in evaluations EVERY WEEK  Make copy of attendance form each week and send in with evaluations.  Ideally you keep the original attendance form through to the booster session and mark off who came to the booster session  Make sure people write first and last names  PLEASE put the names and ages on the Nursery and Children attendance roles

 Why track attendance? (FAQ #19)  What if someone joins late? (FAQ #15)  Who must sign consent and assent forms? (FAQ #14)  Couples must sign their own consent—not their partner’s consent  All youth must sign the assent form  In order to “count,” couples must attend 5 of the 6 classes
