Health Care Reform TripNet is being used by Santa Clara County’s largest public sponsored HMO, the Santa Clara Family Health Plan serving participants in Healthy Families, Healthy Kids, Healthy Workers, Seniors and Persons with Disabilities. Together We Ride - Mobility Management Center for Santa Clara County

Health Care Reform This HMO established in the 1990s, is significantly expanding membership as they gear up for implementing health care reform changes in benefits and eligibility. Transportation is a member benefit. The HMO partners with OUTREACH to seek the most appropriate and most cost effective options. Administrative funds are used for to pay for the transportation.
