Howard County Integration – RHP 14 July 2014

Goal: To provide on-site access at Scenic Mountain Medical Center to behavioral health care while facilitating more opportunities to address both the physical and mental needs of the consumer. This will result in behavioral health providers sharing the same facility, having some systems in common, participating in regular face-to-face communication and functioning with primary care providers in team approaches to patient care. WTC Currently Serves:  44% Medicaid  54 % Uninsured  Remaining percentage having some form of third party insurance or other method of payment However: West Texas Centers is the public safety net for behavioral health, therefore, the target population for this project would include any person in the WTC service area experiencing a psychiatric emergency or episode and accessing the WTC Crisis Hotline.

Project Milestones:  Complete lease arrangements for co-location/integration to include floor plan remodeling design and completion timeframes.  Relocate Howard County Mental Health Clinic/Center to integrated site at th Place; Big Spring, TX.  Develop and implement a set of standards to be used for integrated services to ensure effective information sharing, proper handling of referrals of behavioral health clients to physical health providers and vice versa.  Provide integrated services and coordination of care in DY4 and DY5. Project Goals:  DY4: Of the 425 persons served by Howard County Mental Health Clinic/Center, 72 of those persons will be receiving care at the new location utilizing this project’s integrated health care processes and model to include treatment plans developed and implemented with primary and behavioral health care expertise.  DY5: Of the 425 persons served by Howard County Mental Health Clinic/Center, an increase from 72 to 144 persons will be receiving care at the new location utilizing this project’s integrated health care processes and model to include treatment plans developed and implemented with primary and behavioral health care expertise.  Note: These person numbers are minimum targets and as integrated services progress, more persons will be anticipated to move into the integrated model.

Benefits Of Integration:  Both behavioral health and physical health care will occur at the same location and in some cases the same day.  Increased diversion from emergency department and potential incarceration/law enforcement involvement.  Better communication between providers with enhanced coordination of care.  Increased consumer satisfaction.
