Established in 2015 based on a recommendation in Denver’s Community Health Improvement Plan related to increasing Access to Care Brings together stakeholders from: medical care behavioral health care – mental health and substance abuse treatment public health and other governmental entities social and community services Mission: Achieving Better Health through Collaboration

Tracking enrollment in Denver County, sending enrollment updates to partners. Contact to get updates. Sharing information about outreach and enrollment events and coordinating needs during second open enrollment period Facilitating community outreach and enrollment meetings - Denver stakeholder meeting held July 13, 2015 to develop enrollment plans for specific priority populations: Racial/Ethnic Communities Occupation-Based Approaches Health Care Sites Defining our role in promoting health insurance literacy Matrix of First Access resources developed

9 Specialty Care Access Group
Recommended to form a specialty care referral hub for publically insured, under-insured, and uninsured residents who are patients at Denver’s safety net clinics To include e-consults, in-person referrals, and capacity building for primary care providers Held stakeholder meeting in December to gather feedback on the draft plan Recently received a grant from the Caring for Colorado Foundation ($55K) to fund the development of a more detailed implementation plan for a pilot project - August 2015-February 2016.

10 Targeting High Utilizers Group
Learning the landscape: Existing programs, how they are structured and funded, data sources, mental health and substance abuse connection and need for payment reform, connection with housing Exchanging information and developing relationships between existing high utilizer programs Summit held June 22, 2015 in coordination with CCMU: Learning and sharing about programs in the metro area Panels on payment reform, integrated care, and data/assessment Identifying common challenges Collaboratively identifying next steps to address some of the challenges

11 New Integrated Care Working Group - 1-Year Objectives
Defining Integrated Care for Alliance Members: Develop an operational definition of integrated care to assist Alliance members in understanding the landscape and how they might make practice transformations. Environmental Scan and Identifying Best Practices: Who is doing already integrated care in Denver? How is it working? What are the successes and challenges of those implementing it? How are they paying for it? What are the best practices and tools they are using? Identifying Needs: What is the current need for integrated care in Denver? Are staff in member organizations trained and able to perform their responsibilities and tasks in an integrated care environment? What support do they need to do so? What are the potential resources to support these efforts?

12 New Integrated Care Working Group - 3-Year Objectives
Learning: Provide a collaborative learning environment that shares best practices. Technical Assistance: Develop and share tools that assist member organizations to implement integrated care models in their organizations. Advocacy: Advocate for system-level changes that support the success of these models.
