Programs of All-Inclusive Care for the Elderly (PACE) State Demonstrations to Integrate Care for Dual Eligible Individuals Design Grant The Centers for Medicare and Medicaid Services (CMS) is providing funding through grants and technical assistance to states to develop person- centered approaches to coordinate care across primary, acute, behavioral health and long-term supports and services for dual eligible individuals. The goal is to identify & validate delivery system & payment coordination models that can be tested & replicated in other states.

Programs of All-Inclusive Care for the Elderly (PACE) State Demonstrations to Integrate Care for Dual Eligible Individuals Design Grant The State of Oklahoma has been selected to coordinate the PACE project in demonstrating to CMS that there is truly a need for multiple PACE centers throughout the state of Oklahoma.
