4 Coordinated One team takes care of all your needs Same team follows you through the continuum (home, hospital, nursing home) You don’t get lost in the system. PACE provides transportation to the day center to medical appointments and specialists PACE schedules the home health aides, meals, oxygen etc. PACE will make the appointments with the specialists and tests to provide quality care Low re- admission rates

5 Interdisciplinary All staff sit around the same table to discuss issues and plans of care. Team is made up of a doctor, nurse practitioner, nurse, social worker, pharmacist, dietician case-manager and rehab. Each team takes care of 120-150 participants Preventative care- Staff see participants on a regular basis

6 Customized Each plan of care is customized to the participants needs and situation. Flexibility to offer services that make sense The plan of care is always changing The participant is an active participant in designing the care plan

7 Cost effective Frail elder waiver ($2163) Cost is not affected by the amount of care We do the Medicaid applications if needed We do accept Medicaid only, monthly spendown if over $2163 as well as private pay. You can be 55 years old in the program if disabled. Contracted AL rates (do not pay more than your income)

8 Eligibility 55 years and older Live in Avon, Boston, Braintree, Canton, Dedham, Holbrook, Milton, Norwood, Quincy, Randolph, Sharon, Stoughton, Weymouth Qualify for “nursing facility care” determined by the state. Ability to live safely in the community with supports Payment source: Medicare, Medicaid, monthly spendowns, private pay
