PCS Champs Gift of Love: Support and Referrals

WIC is… WIC is about women, infants and children WIC is a nutrition and breastfeeding program WIC is a program that ask what is on the heart WIC is a point of support WIC is a referral program WIC cares!

Why do we offer referrals and support? Because…they are our friends, family, neighbors, another human being, the next generation…THEY are the future of Arizona.

“E” Assessing for “E” may not give a warm and fuzzy feeling E

E A) Helps identify referral needs B) Can be a factor in growth/nutritional status C) Risk codes are assigned D) All of the Above

Using PCS Probing – Open-ended Questions Active Listening – Reflections Consensus

Using Open Ended Questions When during the appointment?? What to say?? – How to bring it up? – How to probe?

ABC GTHM and D E at the End ABC + E Lab/Growth GTHM Roll With The Flow Which Do You Prefer?

Meet Jasmine Jasmine is in your clinic for the first time.

Invite These doors represent hopes and dreams of mothers all over the world. Choose a door that represents the hopes and dreams you have for your baby.

Mom chooses this door…... You see a lock on this door, if I gave you a little key and you could unlock it, what hopes and dreams would lay behind this door for you baby? Invite Continued

Behind the Door…

Pick the Best Response

Probing…

Follow these two easy steps: – Step 1: Summarize what has been done so far – Step 2: Ask permission or share the reason for what you’re about to ask How to Transition to E

“So, thank you for sharing with me a little about your pregnancy and how you’ve been eating. I have some final questions for you, we ask these of all families, just to see if there is any additional information we can offer. What concerns do you have about safety at home? What about smoking?” Example Transition

Active Listening

Sometimes The Road is Rough Personal crisis – Traumatic events – Financial or living situations – Family issues Personal loss – Loss of pregnancy – Loss of baby or loved one

Active Listening Being present – Showing signs of listening Reflecting and affirming when appropriate – “You’re a good mom” – “You’ve been through a lot” Sometimes…it’s not something you can fix or – A simple “I’m sorry for your loss” is all you need

Meet Kathy Kathy is here for her health check or 2 nd nutrition contact. She is a PG2.

During the Apt You start off the conversation by asking “how are you doing?” She starts tearing up and tells you that she recently miscarried. How would you respond?

Chose the Best Response A)Sometimes these things happen for a reason. B)My friend went through the same thing. Eventually she took home a healthy baby. C)I’m so sorry for your loss. D)Okay, you can still be on WIC, we can recertify you today.

Keeping on Track

Transitioning Back to WIC When Appropriate – May not be!!! – Document in the TGIF 2 Step Transition – Summarize what was just said – Ask for permission or explain next step – Affirm when possible

Example “It sounds like you have a lot on your plate right now. You do so much for your kids! If its okay with you, I’d like to get a better sense of how things are going, nutritionally, to see what you may need from us.”

Making the Referral

Consensus People look to others like themselves to make decisions People often will use “a friend” or “others” to bring up concerns of their own Share what resources other families use

Example Can I share with you what other families in your situation have found helpful? You’re not alone. A mom I talked to yesterday told me about the hotline and that it really helped her. Here is the information.

Questions? Share the love…
