What Will Fill the Gap? 8 Services Provided: - Delivery of groceries, household goods, medicines, and pet supplies by professional caretakers (SAMs) - Treat and care for all clients as if they are an extension of our family - Training for elderly and family members on website and smartphone application SAM will provide independence to the ‘homebound’ population - Serving: Providing goods and services to the front door - Assisting: Acting as an extension of the family support system - Monitoring: Ensuring the household is a stable living environment - And - Restoring - Independence: Bringing back the sense of freedom and involvement - To - Aging - iNdividuals

10 Sign up for Initial Consult Janet and Ester research SAM online Together they select a time that they can both be at Ester’s house They request their preferred consult time with a SAM professional Ester completes a short survey to better pair her with a SAM NAME AND PHONE NUMBER PREFERRED MEETING TIME ADDRESS

11 Initial Consult SAM, Ester and Janet get to know each other SAM will help download the SAM App on their smartphones SAM will setup an account for Ester based on her personal information including any special diets, allergies, etc. SAM will setup payment method selected by Ester & Janet SAM will sign a confidentiality agreement with Ester & Janet SAM will be added to Ester’s pharmacy list SAM will provide training for Ester and Janet Grocery

12 Create Shopping List Ester will select ‘Create new list’ option She will select her groceries from the list of items in the app Ester can select to either search by name or picture

13 Add Medication to Shopping List Ester will select ‘Prescription Medication’ from the services list in the app For a refill, Ester will select a previously ordered medication from the ‘My Meds’ option For a new prescription, Ester will add a photo of the prescription to her Log. Prescriptions must be called in ahead of the shopping list submission.

14 Checkout Ester will submit her list via the app Ester will be asked for a delivery date and time that is at least 24 hours after submission Ester can view what was submitted at any time by clicking on the History button under Services and selecting her submission date

15 Delivery of Shopping List SAM will deliver the requested items within an hour of the requested time SAM will bring all groceries into Ester’s home and help her put them away SAM will conduct a wellness check of Ester and her environment If SAM notices something that seems out of place, SAM will discuss with Ester. If there is still concern, SAM will notify Janet as a precaution

This project aims to alleviate the pressures caused by our aging population combined with changing family structure. Everyone in this room is either affected by this now or will be one day. Providing independence to a growing population of elderly who are depending on fewer caretakers is a winning solution for all families. Our project brings the human element to caring for our elderly. We can sustain a 25% creeping rate on our project based on existing requirements. 17 Summary II
