Student to generate their ethical approval form alongside their research proposal which will be reviewed and considered by a member of academic staff (Project Supervisor) Discussed during SREC - quorate (50% of membership plus one excluding admin support) Member of academic staff to discuss with Independent Studies Coordinator Approved with conditional comments ApprovedDeclined Ethical Approval Submissions Process for UG IS Save application and approval issued via letter or Submission of amendments to be sent through as Chairs action IS Coordinator/Supervisor to advise UG Chair ensures to log onto spreadsheet and informs IS Coordinator IS Coordinator to advise Supervisor Chairs to ensure there are clear and manageable set of protocols for the storage of UG IS forms. It is not necessarily Chairs responsibility to manage the storage (unless they feel that’s the best option to them) Minutes available to UREC Matters arising referred to FRRDC Ethical form deemed minimal risk? Yes No Ask the committee member whose application it is to leave the room during discussions Note: if applicant is the Chair then a Deputy Chair will be appointed specifically to respond to that application

Ethical Approval Sampling Process for UG IS, PGT, PG Research and PG Professional Students All ethical approval forms should be made available on request by SREC. The Chair should select a sample for review at the meeting –maximum of 15% and a minimum of 10 forms. The sample could include applications from each Research Coordinator and forms that had been referred for consideration to the SREC Chair may decide to divide a large number of ethical research forms to committee members prior to the SREC Observations discussed by SREC and feedback to Project Supervisors via IS Coordinators for staff development Chair to log sample review for Annual Monitoring Report purposes
