GP identifies patient at risk of stroke BP maintained below 140/85 Risk factors identified and managed Health promotion Advised on lifestyle modification smoking/diet/exercise Primary prophylaxis TIA suspected Refer to neurovascular clinic (or specialist team) for investigation.
