Single Point of Entry (SPE) for Long Term Care (LTC) What is it? Why do we need it? Who should do it? How will it help the elderly and disabled? How will it impact community organizations?

Background Governor creates Michigan Medicaid Long Term Task Force in October 2004 The task force publishes their final report in April 2005 The Office of Long Term Care Supports and Services and LTC Advisory Commission in June 2005 Governor calls for creation and implementation of SPEs in June 2005

Background Continued… Office of LTC issues a request for proposals for SPEs in November 2005 RFP calls for the creation of 3-4 pioneer SPE sites throughout the Michigan in 2006 RFP indicates statewide rollout of the SPE by 2009

What is a Single Point of Entry? Statewide system that gives consumers access to LTC information and resources through one agency or organization

What are LTC Resources? Medicaid services such as MI Choice Waiver, Nursing Home Care, DHS Home Help, PACE Older Americans Act Services such as Home Delivered Meals, Chore, Home Injury Control RX Payment Assistance Program Housing Options Veterans Services Private Pay Services such as Home Care Caregiver Supports LTC Insurance

The SPEs are Responsible for: Provision of information and referrals through an 800 phone number Provision of LTC options and benefits counseling Determination of functional eligibility for Medicaid and other programs and services Utilization of well trained, experienced staff who are culturally competent and able to assist limited English speaking individuals Provision of Supports Coordination for Medicaid LTC Programs

Supports Coordination is: Assessment of Functional Capacity/Needs Person Centered Planning Care Plan Development Medicaid Service Authorization Monitoring Service Delivery Monitoring Client Satisfaction and other Quality Assurance Tasks Reassessment

Benefits of SPE More Community Based Care Less unnecessary nursing home placement Slower growth in LTC spending Higher consumer satisfaction “One stop shop”

How does dementia fit into SPE? Any person, who has Medicaid, and is accessing LTC services must go thorough the SPE. SPE staff, including Care Coordinators, must be trained in dementia.
