MEMBER OF HEALTH CARE TEAM  Member of the Health Care Team:  A licensed nurse who provides patient-centered care by collaborating, coordinating, and/or facilitating comprehensive care with an interdisciplinary/multidisciplinary health care team to determine and implement best practices for the patients and their families.
