LIC-33LIC-33 Business Address Verification LIC-33 The office and/or shop location for the following licensees must comply with all requirements established in the NYC Zoning Resolution: Plumber, Fire Suppression, Rigger, Sign Hanger, Oil Burner and Elevator agencies. These licensees shall operate a business location in the city of New York at the time of issuance and during the maintenance of their license. Licensees shall submit to the commissioner any changes to their business location within 30 days of such change for verification.

To comply with Worker’s Compensation Law, business must: A- Be legally exempt from obtaining worker’s comp/ disability benefits insurance coverage; or B- obtain such coverage from insurance carriers; or C- be self-insured

Cancellation Cancellation Of Worker’s Compensation This certificate is sent to Department of Buildings directly from NYS Insurance Fund. In order to have your Certificate of Worker’s Compensation reinstated, you must provide the Department of Buildings with an updated certificate dated after the cancellation notice.

WC/DB 100 (7/04) Affidavit for New York Entities with No Employees, that NYS Worker’s Compensation and/or Disability Benefits is not Required. This Affidavit must be notarized and stamped by NYS Worker’s Compensation Board, therefore original form must be mailed or dropped off to our office in order to be updated.

If you have a corporation established outside the city of New York you must submit this application to do business within the five boroughs of New York City. This application must be filed by Department of State. Certificate of Authority

Certificate of Incorporation Must Be Filed by Department of State D.O.S will stamp it and give it a file number.

Continuing Education Certificate Is needed every time you renew your license, No exceptions. The Certificate is good for 2 years.  The following license types must submit this certificate for renewal:  Master Plumber  Master Fire Suppression Contractors  Site Safety Managers

This filing receipt is “ONLY” needed when you become a DBA or change your business name. It will have both the old business name and the new business name on the receipt. Assumed Name Filing Receipt
