What is a Support Plan? A support plan is where the individual decides what is important to them and what they would like to change in their life A support plan will look at what’s working and not working for individuals and what support they need to make things better A support plan does not have to be complicated; it can be as detailed or simple as individuals want People First Programme

Support, Planning Empowerment and Knowledge What is SPEaK? People First Programme This is the name of our partnership between the Local Authority and Service Users Empowerment and the Disability Forum.

Why did we start the SPEaK partnership? People First Programme In response to a grant from the Department of Health We wanted a service user- led process Speak have worked together to develop a new approach to Support Planning.

How does SPEaK work? People First Programme We will recruit 6 Support Planning Champions who will be trained as trainers ask if you want information The trainers will then train service users and carers as Support Plan Enablers

How can people get involved? People First Programme The support plan enablers will then be able to help other people complete their support plans If you are interested please register today
