Cancer Nurse Coordinators Central Cancer Network regional workshop

Faster cancer treatment patient pathway approach that covers surgical and non-surgical cancer treatment focuses on all patients getting quality cancer treatment, faster cancer nurse coordinator roles make up part of the approach to achieve faster cancer treatment measured by three indicators:

Cancer nurse coordinator - funding Budget 2012 allocated $33 million for implementing faster cancer treatment including up to $4 million per annum for cancer nurse coordinators funding allocated to DHBs using the PBFF, adjustment was made so each DHB received funding for at least one FTE

Cancer nurse coordinator - funding Central region 2012/13 funding adjusted   2013/14 and out-years funding adjusted Minimum number of FTE cancer nurse coordinators DHBs can employ DHBs Capital and Coast $164,679 $219,572 2.2 Hawke’s Bay $110,108 $146,810 1.5 Hutt Valley $89,578 $119,437 1.2 MidCentral $116,185 $154,913 Taranaki $76,850 $102,468 1.0 Wairarapa $75,000 $100,000 Whanganui

Cancer nurse coordinator - specifications cancer nurse coordinator roles are part of a strategy that contributes to the aims of care coordination care coordination remains a function and responsibility of the whole health system

Cancer nurse coordinator - specifications DHBs need to: fund new clinical positions focus on the front end of the pathway, not oncology services support streamlining services and referrals between DHBs collaborate regionally when developing and implementing these roles Objective is to: improve the experience for patients, including their family and whānau, with cancer or suspected cancer improve overall access and timeliness of access to diagnostic and treatment services for patients with cancer

Learning from Australia the problem that the coordinator roles are solving needs to be clearly defined before implementing the roles coordinators must have the appropriate training and skills the coordinator roles require the mandate to make changes to systems and processes if they are to improve how patients’ journey through the system without this mandate the coordinator roles will just be filling the gaps in systems and processes need to educate other health professionals about cancer so all health professionals remain responsible for coordinating care, this means that the coordinators can focus on high and complex needs patients

Key messages from Violet Platt aim to make themselves (the cancer nurse coordinators) redundant if you (the cancer nurse coordinator) has done the same thing five times, is there someone else who could be doing it or is there a process issue that needs to be fixed? three circles of influence the patient the team / clinical colleagues strategic / the system need to have good training / education and development programmes develop mentoring approaches and one-to-one peer support

Cancer nurse coordinator - evaluation RFP closes on 18 October 2012 process evaluation how the initiative was implemented and operated, including in-depth assessment of the initiative activities during the development stages and over three years (Dec 12 - Dec15) impact evaluation whether the initiative achieved its objectives, measuring the value added to service users and families, health professionals and regional cancer program development and utilisation. provide Ministry and DHBs with information on how to improve existing services Objectives are to: Evaluate whether the cancer nurse coordinator roles contributed to: improving the experience for patients, including their family and whānau, with cancer or suspected cancer improving overall access and timeliness of access to diagnostic and treatment services for patients with cancer

Cancer nurse coordinator – support develop tools to support the cancer nurse coordinators for example, developing a psychosocial needs assessment tool scope (subject to funding) a National Clinical Nurse Lead for the cancer nurse coordinators case management training programme that includes care coordination processes, practices of case management, and assessment skills and knowledge other suggestions, email them through: saskia_booiman@moh.govt.nz
