How can you refer to us? The goals of the FIT team are to: Identify frail older people (>75 years) in the Emergency Department Initiate HSCP Comprehensive Geriatric Assessment in the ED using a standardised assessment process. Develop an ethos of ‘every hour counts’ for the frail older person in ED Transfer suitable patients direct to the appropriate destination as soon as possible Develop a HSCP treatment and care plan in conjunction with the medical and nursing team Focus on expediting discharges To refer to FITT please bleep the Team Leader on #3039 We are available 9am-5pm Monday-Friday
