Protocol for Addressing Homelessness II. Identification: Protocol for Addressing Homelessness Parent and/or Counselor Complete the Louisiana Student Residency Questionnaire Form If Questions # 1 & 2 are checked YES COUNSELOR should conduct assessment with student & parent Counselors Signature Needed on the form!!!!!!

Continuation of Protocol for Addressing Homelessness Counselor should assess the case by completing the SJBP McKinney-Vento Evaluation Form & McKinney-Vento Confidential Referral Form Write a detailed explanation in the Comment Area including name and number of person(s) family is residing with Counselors Signatures are in designated areas on all forms

A. Defining Homelessness Doubled Up – Living together due to economic hardship, can’t find affordable housing, eviction, foreclosure, unemployment, domestic violence, health care, mental illness, or long term poverty Sheltered (examples: transitional or emergency) Unsheltered (examples: cars, parks, abandoned building ) Hotel/Motel Unaccompanied Youth – A child who is NOT cared for by his/her BIOLOGICAL Parent. Migrant or Immigrant students Reference to page 7 in the Education of Homeless Children & Youth manual.

Continuation: Enrolling Students in School Disputes & Disagreements *State & schools districts must abide by McKinney-Vento. Reference to page 21 in the Education of Homeless Children & Youth manual.

IV. UNACCOMPANIED YOUTH A. Definition: Is a Student residing with someone who is NOT a parent or court –appointed legal guardian abandoned by parent(s) runaway fled from home due to conflict or abuse (domestic violence

Roles & Responsibilities of School Level MV Contact Conduct Beginning-of-the-Year Faculty In-service Serve as the School Level MV Resource Person Coordinate the completion of all required Identification & Enrollment forms Complete Beginning-Of-the Year MV Identification Status Review Complete and return all SJBP required forms Complete Monthly MV Monitoring Reports Counsel Parents on MV services and policies

Roles & Responsibilities of District MV Staff Homeless Liaison Advocate for homeless students Review and approve McKinney- Vento referral forms Monitor monthly school reports Monitor student’s report card data received from counselors Collaborates with teachers and parents to improve academic achievement Maintain relationships with community agencies Conducts professional development for school personnel Disperse school supplies, uniforms and other needed items Maintain student files Attend coalition agency meetings Assist with issues of obtaining enrollment documents Assist with transportation concerns
