A workshop of two halves A common understanding of safety Safety in the context of frailty

Where does safety fit? STEEEP Principles* as outlined in the IOM report, Crossing the Quality Chasm: A New Health System for the 21st Century S – Safe Care: The patient's safety comes first. T – Timely Care: Patient care will be delivered in the most timely manner possible. E – Effective Care: Patient care will be based upon the best science available. E – Efficient Care: Patient care will avoid waste of time, money, and resources. E – Equitable Care: Access to care will be provided to all in an equitable manner. P – Patient-Centered Care: Patients will participate fully in care decisions.

How do you know you are safe? What information and data do you use to think about safety? – write on post-it notes for later

Avoiding sleepless nights Safer Salford will learn from the latest thinking about safety in health and care, applying this to ensure care delivered in Salford is the safest in the UK. Measuring and Monitoring of Safety Framework (Vincent et al, 2013)

How are we completing our risk assessments? Has this changed over time? Are there other factors we need to be aware of? Measuring performance Measuring safety Improving safety What happened? How did this happen? Triangulating information to support learning

Are we mature? Think about your role in your setting on a day-to-day basis Consider how often and in what format you use data and information related to safety Highlight the section which best matches YOUR experience Collect individually on paper and then get whole group to assess on the whiteboard. In second workshop give time to complete individual assessment and review whiteboard, but not in depth.
