PDSA Template Owner: Date: 1. Plan 2. Do What change idea are you testing? Where, when, with who?. Run the test. PLAN ADJUST CHECK DO 4. Act 3. Study Analyse or review how things went. Look at outcome measures. Speak to patients and workforce about their experience. Did you achieve the aim of the change idea? If not, why not. Use Root Cause analysis to see why things didn’t go well or exactly as you thought they would (why, why, why, why, why) Make the changes to the process and try again. Changes could be : Adopt Adapt Abandon Idea (a new change idea)
