San Mateo County Fall Prevention Task Force The Task Force informs, collaborates and raises awareness to prevent falls among older adults living in our community. Mission Our stakeholders – older adults, member organizations, funders, hospitals, physicians and other care providers – expect us to: Stakeholders …refer fall prevention resources to those who need them, and ensure the information we provide is up-to-date …demonstrate the impact and results of what we do by collecting and analyzing data …understand the needs of our community related to fall prevention …demonstrate good stewardship of our financial resources Establish a permanent source of funding to support our operations and activities. Increase medical providers’ use of fall risk assessment tools and referrals to fall prevention programs and services. Integrate fall prevention into education curriculum for many types of medical professionals. Document the impact and outcomes of our work. Key Themes Provide community-based fall prevention education, outreach and referrals for key stakeholders. Develop a formal structure for leadership and membership of the Task Force. Promote fall prevention through policy and awareness-raising at the city and county levels. Expand fall prevention programs and service delivery through community partners. Strategies Determine funding needs Identify organizations with aligned incentives Create long-term funding agreement Create bylaws describing leadership positions and terms Consider incorporation Give STEADI toolkit to primary care providers Develop a resource algorithm for referrals Connect with partner organizations Identify programs to support through partners Identify certificate and degree-granting programs Partner with academic programs to develop content for curricula Create internship opportunities Serve as experts on changes to built environment Implement evaluation tools Track morbidity and mortality data Quantify outreach efforts Distribute educational brochures Improve website Increase social media
