MOCH (Medicines Optimisation in Care Homes) Pharmacists
What is MOCH programme NHS England found that care home residents don’t have the same access to a pharmacist as patients in their own home. MOCH allows specialist pharmacists to provide support for care home residents by working as part of the care team. Pharmacy technician to ensure efficient supply and management of medicines
What we aim to achieve Enhance the quality of care through better medicine use Reduce the risk of harm from medicines and support safer medicines systems Release resources through medicines optimisation and waste reduction, reduction in hospital admissions and release of carer’s time.
Partnership working We will work with Care Home staff Care Home Support Team Pharmacy colleagues in GP practice, community pharmacy and secondary care. GPs Specialist teams and Geriatricians Gloucestershire Care Providers Association

Examples of what we do.. Provide training for staff Advise on medicines policies, homely remedy protocols Check ordering systems, reduce missed doses Enable self – administration of medicines where appropriate Advise on the use of adherence aids
