The Task: 1. You need to identify the stages of Chronic Kidney Disease (CKD) and discuss how you would know each of the stage. Think about the subjective and objective data that you would need to make your assessment. What evidence would indicate each of the stages…think about the signs and symptoms. 2. You then need to identify the stage of Glenda’s CKD and how you came to that conclusion. What evidence do you have (this information is in the moodle site and the video) that enables you to identify this stage. 3. Now you need to consider how you would manage Glenda’s CKD

Assess, diagnose, plan, implement and evaluate The Nursing Process Assess, diagnose, plan, implement and evaluate This is what you can use to ensure that you cover all of the care glenda requires (these are you guiding principles for any patient) You make an assessment and with that data you make a diagnosis. You then plan and manage the care and finally evaluate it to see if it worked!

How to start Start with an introduction of what you plan to do within this essay then define and describe CKD Apply this to your scenario which is Glenda Once you know the stages of CKD you should be able to work out glenda’s condition

Glenda You will need to provide information about glenda – try to summarise the case study. ensure you include relevant information with regards: signs and symptoms her medical history social history medications results of her physical examination or nursing assessment what was her final diagnosis? treatment nursing care final outcomes Glenda presents to the clinic in stage 5 renal failure and is in need of urgent dialysis. Include treatment options, medications, diet, cultural and social support. You do not need the GFR as there is enough other indicators pointing to CKD stage 5 in the data.

Conclusion – summarise everything you have written about! Management of care consider how glenda’s care was managed at each stage who were the health professionals that were involved and what were their roles – do you think there should have been any other health professionals involved? why? what about health education? what about patient centred care – was glenda involved in the decision making in relation to the care and treatment received. were her choices respected…what about autonomy? Would you make any other recommendations? Conclusion – summarise everything you have written about! An excellent Journal Article is: Redmond & McClellend (2006).
