Are you a PCBU or a Volunteer Association? Are you and all others involved in the business or undertaking acting on a voluntary basis? The organisation is a PCBU and will owe duties, including to all its workers (paid or volunteers) No Yes No Are you working for a community purpose? Yes The organisation is a VOLUNTEER ASSOCIATION. It is not a PCBU and does not owe any duties under the act Yes Do any of you employ someone to carry out work for the business or undertaking? No

Risk Management Matrix   Level of Risk Likelihood (chances that harm could occur) 5 Almost certain 4 Likely 3 Moderate 2 Unlikely 1 Very rare Risk Score Consequence (potential outcome) The potential outcome would be Catastrophic (Fatal) 10 9 8 7 6 High Risks with a rating of 8, 9 or 10 The potential outcome would be Major (Serious harm) Risks with a rating of 5, 6 or 7 The potential outcome would be Significant (Time off work/unable to continue usual daily routines) Low Risks with a rating of 2, 3 or 4 Minor (Medical treatment) Significant Risks Significant risks are risks with a rating of 4 or more. Significant Risks must be; Eliminated Isolated Minimised The potential outcome would be Insignificant (Minor first aid) Adapted from the Risk Management Matrix in the Home and Community Support Sector Standard Audit Workbook
