Safety Guidelines Eligibility and Insurance Statement – with insurance info Concussion Form Physical – good for 2 years Forms Needed ALL athletes/managers for EVERY sport

ASB Card - $15 – paid once a year Sports Fee - $10 ($26 for football) – paid each season District Fee - $25 – paid each season – pooled within the district Fees

Academics come first Attitude is everything Show good character Sportsmanship ALWAYS Come ready to learn and play Follow school rules Follow WIAA rules Our expectations for athletes

An athlete with 1 F Must attend 90 minutes of Study Table a week Can still practice and play in games An athlete with 2 or more Fs Must attend 90 minutes of Study Table a week Can still practice, but cannot participate in games Grades MUST improve within 3 weeks, or the athlete will be removed from the team Grades

Academics come first Attitude is everything Show good character Sportsmanship ALWAYS Let the coaches coach Ensure your athlete is at practices and games on time and is picked up on time afterwards (5:10 pm or 10 minutes after a game at the LATEST) If there is a problem, contact the coach first, then the AD, then the principal Our expectations of parents
