‘ Almost 1,000 elderly people a day are being admitted to hospital needlessly…’ ‘…hospitalisation could potentially have been avoided had the person been better looked after..’ ‘there’s problem of older people stuck in hospital and unable to go home..’ Age UK, 2018

Objective The Healthy Housing Hub is trying to solve the increasing problem of avoidable hospital admissions along with improving quality of life for the people we help by making homes healthier.

Who are we? Part of Derby City Council Strategic Housing Services. Support Derby City’s JSNA and H&WB strategy Our partners include: Fire Service, Police, Hospital Discharge teams, EMAs, First Contact, OTs, GPs and Community Matrons. Funding from the Better Care Fund.

Why is there a Healthy Housing Hub? To reduce avoidable hospital admissions. Support vulnerable people. Maintain mental and physical health by improving homes.

Risks - Clutter on the floor could lead to trips and falls. - Fire hazard.

Risks Personal hygiene health risk from unhygienic conditions

Risks Personal hygiene health risk from unhygienic conditions. Risk of trips, falls, scalds and fire from hoarding; risk increased in clients with restricted mobility.

Risks Risk of falls from lack of handrail. Risk of falls from clutter on the stairs.
