Who Are We? We are a team of health professionals offering a service to children and adults with Learning Disabilities. We work closely with other agencies including Social Services. The service is available to anyone living in the Scarborough, Whitby and Ryedale areas.

Who's in the team? Adult Nursing Team –work with people with a Learning Disability aged 18+ The “Children’s Health Team” -Working with children (aged 0-19)with learning disability and developmental delay. (No LD psychiatry – CAMHS) Occupational Therapist – 18+ Speech & Language Therapy-18+ Symbolic language Advisor works with Adults, provides symbolic support to the Children's Team.

Who's in the team? Psychiatry -18+ Psychology – works with Adult and Children's Strategic Health Facilitation Forensic Community Team – 18+ Physiotherapy- 18+ Admin Team

We offer training and support around … Positive Behaviour Support Mental Health Problems Health Assessment/Health Action Plans Communication assessments Independence Skills/OT assessments Epilepsy Training/Support (Neurology) Sexual Health

We offer support to… Support with medical appointments if someone lives independently. Assessment of daily living skills Aids and adaption's – bath seats, walkers etc Sign posting to other services

Training Health Promotion Advice/Health Facilitation Anxiety management groups/sexual health Learning Disability Awareness Positive Behaviour support Dementia

Referrals The team accept referrals from GP’s, Social care, self referral, carers, family members etc. . Please see criteria hand-out. All referrals are sent to our Single point of access email TEWV.NYLDreferrals@nhs.net

What we don’t do….. Assessments for housing –Stonham Housing, MIND, Foundation Housing and DAG (Disability Action Group) Social care assessments- day activities, care support – Adult and Community Services (Social services) Financial assessments- Adult and Community Service
