Goal: identify immediate psychological and social risk that contribute to crisis and risk of child maltreatment. 2.Contractor advises case manager on weekly basis, regarding the familys status. 3.Contractor advises County DFCS office within five days if is unable or unwilling to service a particular family and states the reason for this decision.

24Sunday, June 01, 2014 Summary The Serenity House Personal Care Home, Inc. provides supportive living for persons who are mentally and/or physically challenged and have substance abuse problems. Clients are assisted with individual mastery of living skills with anticipation of being reintegrated back into society with maximal independence. We employ Social Workers, Certified Addiction Counselors, CNA's and trained care providers. Serenity House Personal Care Home has been in the business of providing assisted living since January 1998, after being incorporated into the Serenity House establishment and obtaining a state license to operate. Clients of Serenity House Personal Care Home include individuals with substance abuse, or mental and physical challenges. Care is provided around the clock for all clients of Serenity House Personal Care Home. SHPCH specializes in Early Intervention, Parent Aide and Homestead Services
