WHAT ARE OUR GOALS Increased Skills or Income 100% of households will develop safety plans showing understanding of domestic violence risks and responses by exit of program. 100% will develop goals that outline their needs and focus on their strengths 75% of participants will complete a personal finance education program 11/15/2018

GOALS Residential Stability 75% of households will obtain permanent housing at exit 65% of households that obtain permanent housing will remain housed at 6 month follow-up 11/15/2018

GOALS Greater Self-determination 75% of participants will not return to their abusive partners 75% of participants will by exit complete at least 50% of their goals (Noted as “ALL” or “MOST” in Service Point) 11/15/2018

Eligible Participants Survivors and their children MUST always be escaping DV Unable to obtain shelter because of barriers or SHELTER IS FULL Have been in Shelter for more than 60 days and due to barriers have been unable to find housing. 11/15/2018

ADDITIONAL BARRIERS Are teens with children Have language/cultural barriers Have mental health problems, developmental disability, substance abuse history/issues Rental history problems/poor credit history Have no financial resources Large families, older male children, or children with special needs Are immigrants or refugees with issues related to this status Have a criminal history that limits their ability to find stable housing 11/15/2018

Verification of Homelessness You must have a signed County Form in the file. You must have documentation of Domestic Violence if this is the source of homelessness Documentation can be: FAPA Signed statement from client Police Report 11/15/2018

Safety Planning WHEN should you do it? What should it include? Is there a required Format? How do you record for HUD that a plan was done? 11/15/2018

STENGTH BASED ASSESSMENT Strength based case management focuses on the strengths of the participants, their own goals, and the tools they already possess to accomplish those goals. It is generally easier to focus on their needs and not their strengths 11/15/2018

What is a strength? Life Experiences - Survivor? Property or Assets? Health? Children and Parenting? Desire or Determination? Faith? What other strengths? 11/15/2018

How Do You Use These Strengths in Planning? Goals/Case Plans Weekly Meetings Case Notes To Encourage the Clients How else could you use a participants Strengths 11/15/2018

Confidentiality and Participant Rights You may have a statement of Confidentiality that protects you in the file but it is not required. You must have a statement that protects the participant You must include a statement that outlines a participants right to be treated with respect and dignity. They must have the right to review their case files and to file a grievance. 11/15/2018

Income Verification Must be Completed. If they have not income they must make that statement in writing and attach it in addition to signing the form Must obtain income proof Must be at least in 50%percentile Should be updated quarterly 11/15/2018

Rent Calculation Worksheet (What a Pain) You must do it even if the participant dosn’t have income Some new guidelines from HUD as of August 2008 You must apply these guidelines from November 1, 2008 forward 11/15/2018

So Now the Process Is Fair Market Rent is the value after calculating Utility allowance. So if the Utility Allowance is $50 Fair Market is $750 Maximum rent would be $700 You can not pay more than $700 in Rent DISCUSSION !!!! THERE IS MORE 11/15/2018

Discussion You can pay the utility allowance directly to the client (Not always a good plan) You must take all of the rent calculation worksheet into consideration You can no longer just use the Fair Market Rent as a guide for maximum expenditures in most cases 11/15/2018

Habitability Standards USE the Required Form Must be Done Prior to Lease Agreement Make sure you check each component If Public Water then it is SAFE You must inspect public areas such as laundry, shared kitchen, 11/15/2018

THE REAL HEART OF THE FILE Intake and Contact Information Assessment Case Plan/Goals (should be reevaluated every 90 days minimum) Case Notes – see “How to write case/ contact notes” FOCUS ON STREGTHS 11/15/2018

MORE REQUIRED ELEMENTS Agency contract Lease agreement Any letters or correspondence List of expenditures Any other housing related information Lead Based Paint Notification 11/15/2018

EXIT DOCUMENTATION Must have an exit summary that includes Why they left Attach a copy of any exit survey they may have completed Date of Exit and follow-up dates Letter to participant. Please include a copy of the Grievance procedure. 11/15/2018
