A Patient’s Guide to Inpatient Rehabilitation at Mount Sinai

Rehabilitation Team: Patient Centered -Patient and Family -Physiatrist -Consultant physicians -Nurse -Outreach Coordinator -Recreation Therapist -Speech therapist -Physical Therapist -Occupational Therapist Neuropsychologist -Social Worker -Case manager

Your Role in Rehabilitation Your active participation is KEY Work with you team to develop and achieve goals Your willingness to learn and apply skills enables you to maximize the benefits of your program You must be able to direct your care As you improve, you are expected to take responsibility for your own care and manage your activities of daily living while you are still on the unit Additionally primary caregivers are expected to be trained on assisting you with your activities of daily living (ADLSs) and are required to participate with you in the room

Your Customized Program A Physiatrist evaluates you and prescribes your treatment program You are involved in an interdisciplinary team assessment including nurses and therapists You will be provided with a therapy schedule every day by 9:00 AM You are required to participate in at least 3 hours of therapy a day this includes Physical, Occupational and Speech therapy Your therapy schedule is subject to change in order for us to accommodate medical testing and nursing needs

Life on the Unit You are expected to be up and out of bed for a large portion of the day participating in various therapy We recommend wearing comfortable gym clothes and sneakers Your room has a bulletin board for information Your patient binder has additional information on your therapy program and nursing needs

Daily Routine* Time Activity 5:30-7:30 AM Morning nursing care routines 7:30-8:00 AM Breakfast/ADLs with OT 8:00-9:00 AM ADLs , Medications 9:00- 12:30 Individual and group therapy 12:30-1:30 PM Lunch, Medications, Support Groups 1:30-4:30 PM 3:30-4:30 PM Recreational Therapy 5:30- 6:00 PM Dinner 6:00-9:00PM Evening activities, Visiting hours 9:00 PM End of visiting hours 9:00 to 11:00 PM Routine medications, bedtime * Note schedule here may in no way mirror your particular schedule.

Your Responsibilities Be an active and responsible participant in your rehabilitation program Keep your scheduled therapy appointments Participate in training sessions and education classes about your care Practice what you learned with the appropriate level of guidance Respect the privacy of others Respects the rights and beliefs of other patients and staff Refrain from noisy/disruptive behavior

Visiting Policy Family and friends are an important part of the rehabilitation process Visiting hours are between 9:00 AM and 9:00 PM Family members are welcome to be active participants in therapy sessions when appropriate
