2 Current Forsyth County Services
The Forsyth Center Provides the following 8a-5p Adult/Child Mental Health and Substance Use Services Advance Access Services (Walk ins) Outpatient Individual and Group Therapy Psychiatric Evaluations Medication Management

3 Behavioral Health Urgent Care
What is it? The Behavioral Health Urgent Care provides 24/7 advanced access services to children and adults seeking mental health and/or substance use services. As an alternative to Emergency Room and Inpatient Psychiatric Hospitalization, adults have access to a 23- hour observation chair unit that will allow patients to stay up to 23 hours for observation, further assessment, referral to a higher level of care, and discharge planning, based on clinical necessity. The unit is staffed 24-hours a day by nurses, licensed clinicians, and certified peer support specialists.

4 Who can present to the BHUC?
Advanced Access (Walk-ins) Anyone needing a Comprehensive Clinical Assessment (Children and Adults) Children will not be able to stay in the 23 hour chair. 23hr. Chair Inclusionary Criteria Any patient in custody/care of EMS/LEA IVC/Voluntary Detox (subject to detox bed availability) 23hr. Chair Exclusionary Criteria Medical acuity Aggressive

5 What services are provided while in the 23 hr. Chair?
Comprehensive Clinical Assessment Nursing Assessment Peer Support Individual/Group Services Individual/Group Therapy Vital and Symptom Monitoring Resource Assistance Discharge Follow Up Mobile Engagement/Crisis can provide follow up until an outpatient appointment is attended and will follow up if the client misses their scheduled appointment

6 Mobile Engagement/Mobile Crisis
Mobile Engagement is a 24/7/365 community service providing evaluations, triage, support, access to mental health, developmental disability, and/or substance use services, and impatient diversion.  Mobile Engagement can respond when; An individual appears to be suffering from a mental health/substance abuse/developmental disorder The person and/or family are experiencing an acute, immediate, non-medical crisis The person and/or family has insufficient or severely limited resources or skills with which to cope with the immediate crisis The person and/or family member exhibits impairment of judgment and/or impulse control and/or cognitive/perceptual difficulties The person is intoxicated or in withdrawal and in need of substance abuse treatment and unable to access services without immediate assistance.  Mobile Crisis Dispatch #:

7 Other Services within Forsyth County
Assertive Community Treatment (ACT) Serving adults in the community with severe persistent mental illness to decrease hospitalization and increase independence Community Support Team (CST) Serving adults in the community to assist in achieving rehabilitative and recovery goals. Psychosocial Rehabilitation (PSR) Serving adults in a 5 day a week clubhouse setting to increase overall skills such as activities of daily living and decrease hospitalization while increasing independence Intensive in Home (IIH) Serving children in the community that need a higher level of care than basic outpatient and a preventive to out of home placement
