The JCAHO standards of quality of care of heart failure (HF) are defined as follows: HF-1, discharge instructions: patients are discharged with complete written instructions or educational material; HF-2, left ventricular function (LVF): hospital record contains documentation that LVF was assessed or is planned; HF-3, eligible patients with left ventricular ejection fraction less than 40 or LVF consistent with moderate or severe systolic dysfunction are prescribed an angiotensin-converting enzyme inhibitor; and HF-4, patients with a history of current or recent smoking cigarettes are counseled about smoking cessation.
