CHW Fundamentals Completed Training Program Overview and Highlights CHW Fundamentals – Montana is a standardized Program designed for individuals interested in becoming a Community Health Worker (CHW) or for those already in CHW positions. It is an instructor/preceptor-guided Program; 80 hours in length. The Program consists of 4-15 hour Training Modules and 1-20 hour CHW Agency/Facility Supervised Experience Module taken at the completion of the 4 Training Modules. Students taking CHW Fundamentals are expected to achieve a PASS in each Training Module attempted. Additionally, during the Supervised Experience they are to successfully perform all expected CHW skills, submit all assignments, complete a Reflective Practice Journal, and present a CHW Case Study.

CHW Fundamentals Videos Introduction Video – FULLY COMPLETED Focused interviews of 4 working CHWs  embedded in the summary of each module. Emphasis on their role at the agencies in which they work and best practice/handy tips on the content of that module - NEARLY COMPLETED Module Vignettes – 2-3 minutes FULLY COMPLETED   Module 1 -  Compassionate Communication. Student views the video and responds by critiquing the communication...what was good, what was not good, and what could be done to improve.   Module 2 - Interviewing a client on a first home visit. Student views the video and responds by critiquing the interviewing skills...what was good, what was not good, and what could be done to improve.   Module 3 - Priority Setting and Work Planning - role play between a supervisor and CHW regarding a client referral...client  background, needs, and the goals for the care are provided.  Student views and then identifies priorities and develops a work plan.   Module 4 - Motivational Interviewing - Student views the video and responds by critiquing the motivational interview...what was good, what was not good, and what could be done to improve.

CHW Fundamentals Module 5. CHW Agency/Facility Supervised Experience At the end of this Module, students will: 1. Satisfactorily complete the 20 hour supervised experience. 2. Complete all required CHW activities. 3. Submit all required assignments and the CHW Reflective Practice Journal. 4. Successfully demonstrate CHW skills. 5. Present the selected CHW Case Study. 6. Complete the Module Evaluation of Supervised Experience.

CHW Fundamentals Implementation – Discussion Self-Paced or Instructor Guided (Modules 1 - 4) Online Learning Platforms

CHW Fundamentals Instructor/Preceptors – Supervised Experience CHW Instructor/Preceptor Expectations A CHW instructor or preceptor is assigned to and has full accountability for CHW students in partnership with the facility/agency. The instructor or preceptor provides direct supervision of CHW students on-site and are responsible for orientation to agency policies and procedures, providing guidance, monitoring progress, giving constructive feedback, evaluating students, and supporting CHW students during their Supervised Experience. The CHW instructor or preceptor must be a practicing community health worker, care coordinator/care manager, social worker, community health nurse, public health nurse, or related provider. Satisfactory completion of the Montana CHW instructor Training is required.

CHW Fundamentals Instructor/Preceptor Training and Certification - Discussion Online training provided for CHW Instructors/Preceptors. Successful completion results in a certificate. When will the training be ready for implementation? How will CHW Instructors/Preceptors be monitored?
