after you have watched the presentation. Thank you for watching This presentation is about a new joined-up service for people with diabetes. We would like to know your thoughts and opinions on the new service, after you have watched the presentation. Thank you for watching

Living with diabetes can sometimes be difficult and confusing

There seems so much you have to think about

Knowing what I can and can’t eat Doing more exercise Maintaining or losing weight Checking my feet Checking my eyes Checking my blood sugar Checking my kidneys Checking damage to my nerves

It all seems very complicated

Life seems to be nothing but appointments to different professionals all in different places!

It would be so much nicer if…… All the professionals that looked after my Worked together as part of a……..

Team Putting me at the centre of my care

Well….. the new service does exactly that………….. really?

The new service will mean ‘the team’ which includes you (the patient) work together to make sure all the things you need to get checked are done by your GP. And the team of people supporting them. Only when it is absolutely necessary, perhaps in an emergency or when you need special equipment, will you need to go to hospital. Your GP will be able to contact the team, by phone or by email or make an appointment for the team to come to your surgery.

What will be different for me? 1 More of your care will be managed by your GP. 2 Your GP will have more expertise because other specialists will support your care with your GP. 3 Your GP will be using the phone and emails to access specialist help when you need it. 4 The service will be more responsive and closer to home. 5 There will be more, consistent information available to patients at their GP practice. 6 You will not be referred into the hospital unless absolutely necessary.

But I have some questions This is not about buildings, this is about all the different health professionals that currently look after you, making decision together, providing care closer to home with you and your GP. All patients will be able to access this new service. We will be providing new specialist training for all practices. GPs will also be working with consultants so these skills will be shared We believe that this service puts patients at the centre of their care Where will this specialist team be based? Are all GPs signed-up to the service? What about the expertise of my GP to care for me? How will this new service help patients?
