Leominster - slides and feedback Event – 1 August 2017

Leominster told us Care delivered in local area Information is key Good access to diagnostics, opticians and pharmacy Good voluntary and community services with potential to utilise more Traditional model of NHS not sustainable GP surgeries could be a central hub Do more locally, such as outpatient appointments Limited mental health support There is no integration with social care Improve connection between services Information is key Insufficient information given by the NHS Between my doctor and hospital People’s understanding of conditions and self-care Improve navigation and signposting Access to health records across the system and for the public Awareness of who to contact when Trustworthy information More than information available through internet Person-centred care Know me, not my condition End impersonal care Share results with patients Recognise that people are reluctant to seek support Access is poor Parking is a challenge for NHS appointments Length of appointments are too short Poor access to NHS dentist Lack of support with mental health Access to equipment is difficult Mobility is a factor Healthy Lifestyles in your Community Develop self-help groups and help people to identify others to form support networks Health-checks for all Issue guidance on prevention Recognising carers Improve quality Standards of care are generally good Professionals are interested in caring for people There is duplication in tests and appointments (based on each condition rather than the person) Follow-ups are not routine Community rehabilitation is not sufficient

What’s important to Leominster Improved access Long-term support with mental health Recognition & support with mobility More care at home rehabilitation Develop multi-disciplinary teams Training & support for carers After-care Social care Virtual ward Early help to keep well Involve me in my care Team work Access to records & results See the person, treat holistically Continuity of care Address isolation and loneliness Health checks

Guide people through system What’s important to Leominster Guide people through system Better communication Key workers Use local clubs to deliver signposting to appropriate services Within the NHS Build up WISH With the public about the NHS Advice on self-management Focus on enhancing skills Tools to check my health Accessible mental health self-help Local self-help groups

Key Issues for Leominster Need for integrated health and social care system (LA, CCG, Providers and voluntary) Guide people through the system Lack of infrastructure for increasing care at home Improve access to named person in primary care Improve coordination and communication between different sectors of health and care system Seen as a person and being involved in my care Promote telecare and assisted technology

What Leominster asked us Q: Would you like everyone to come to these events? A: Yes, these events are for the public to participate in this work on community health services. We want to use people’s experiences of care to inform plans for improvements. Q: Why is Leominster Hospital run by a GP practice? A: Wye Valley NHS Trust run Leominster Community Hospital with medical cover provided by Mortimer Medical Practice, based in Kingsland. Q: Who are Wye Valley NHS Trust? A: Wye Valley NHS Trust is a NHS provider of acute (hospital) and community services in Herefordshire. Q: Why is there a difference in way things are done between different GP surgeries? A: GP surgeries are independent clinical services that are organized to meet the need of their patients. As of 1 April 2017, the CCG became responsible for commissioning primary care. The CCG are working with all practices to achieve similar ways of working. As patients, you can influence your surgery through the Patient Participation Group. Across practices, GPs and Practice managers come together to share good practice and learn from each other.
